Fill them out either online and e-mail them to info@magickhands.com or print them out and bring them with you to your first session.
It is highly suggested that Ryan be notified of any pre-existing medical conditions prior to the appointment so he can prepare for the session properly.
Please arrive at least 10 - 15 minutes prior to your scheduled appointment. Ryan prefers to give clients their full time, however late arrivals may have to have their sessions truncated due to tight scheduling. In order to avoid this, we emphasize giving yourself a few minutes leeway for the unforeseen delays that arise in ones day.
